Registered Nurse/Case Manager: Duration:13 Weeks Shift: Day 5x8-Hour (08:00 - 16:30) Description: Registered Nurse Case Manager coordinates patient care by developing, implementing, and evaluating personalized care plans. They work closely with patients, families, and healthcare teams to ensure effective treatment and smooth transitions between care settings. RN Case Managers monitor progress, manage resources, and advocate for patient needs. Their goal is to improve outcomes while promoting cost-effective, quality care.
