Job Description:
The Registered Nurse Case Manager plans, organizes, and directs
hospice care, emphasizing community health education and resource
utilization.
Performs comprehensive initial and ongoing assessments to develop
and revise personalized care plans involving patients and
families.
Provides professional nursing care, administers medications, and
educates patients on self-care and comfort measures.
Coordinates patient care as RN Case Manager, communicates with
physicians and community providers, and documents care activities
accurately.
Supports emotional needs, collaborates with interdisciplinary
teams, and participates in quality improvement initiatives.
Supervises hospice aides, ensures equipment readiness, and
maintains professional growth through continuing education.
Requires nursing licensure, recent acute and home care experience,
strong communication, and physical ability to support patient
needs.
